    The federal Jeanne Clery Disclosure of Campus Security Policy and Campus Crime Statistics Act, as amended, requires colleges and universities to provide information related to security policies and procedures and specific statistics for criminal incidents, arrests, and disciplinary referrals to students and employees, and to make the information and statistics available to prospective students and employees upon request.  Federal law requires institutions with on-campus housing to share with the campus community an annual fire report.

    Please list the name of your primary mentor whom you have asked to send a letter stating their commitment to you as your mentor. The letter should also address their commitment to your career. We recommend you notify them to watch for the email to ensure their responsiveness.

      Personal statement: Attach a one page personal statement detailing your commitment to a research career consistent with the goals of the CTSA TL1 program.
      Research description: Attach a one page description of your proposed area of research.

      Be as specific as you can about the area in which you plan to study, your educational and research experiences that have led to your interest in translational research. The content and quality of your statement are crucial elements of your application.
